Transitional Care Management (TCM) programs are intended to assist patients in their transition from inpatient to community care. Following a hospitalization or other inpatient facility stay, a patient may require appropriate care and medical assistance to cope with the new diagnosis or therapy adjustment. The physicians and the patient’s family are responsible for this transitional care.
As per the transitional care management (TCM) reference provided by the US Centers for Medicare & Medicaid Services (CMS), funding for TCM is restricted to patients whose conditions need medium- or high-level decision-making. This covers time spent helping patients with everyday duties and arranging resources for their particular medical or psychosocial needs.
