Despite use of intra-arterial vasodilators, deep sedation with propofol, and gentle retraction, the sheath could not be removed and eventually broke requiring surgical removal. Discussion This case demonstrates severe radial artery spasm causing sheath entrapment that required emergency surgery for sheath removal.
